Complex Care Consultations

Complex Care Consultations (CCC) are designed to assist individuals, 65 or younger, who are currently suffering from multiple chronic health conditions better manage their conditions and improve their overall quality of life. These consultations are longer than a regular doctor's appointment to allow for more time for discussion of health concerns and treatment options.

Individuals can refer themselves or be referred by a healthcare professional.

Consultation Overview

  • Review of the individual's medical history and their current medications.

  • Discuss with the individual about possible underlying or unaddressed issues that should be dealt with, possible medication changes and any needs that are currently not being met (physical and/or occupational therapies, mental health, self-care, etc.).

  • Creation of a Treatment Plan to address all issues discussed. This includes medication changes (if any), referrals to other healthcare professionals and when a follow-up consultation will be done.

  • A paper copy of the Treatment Plan will be given to each individual by a member of the CCC Team.

  • Follow-up consultations can be scheduled anywhere from one (1) to twelve (12) months after the initial consultation. The time frame between consultations will be determined on a case by case basis.

  • The Chronic Disease Management (CDM) Nurse will monitor individuals through the course of their Treatment Plan.

  • *NEW: Ankle-Brachial Index – a test ordered by your physician, can now be conducted in the Edson PCN office only, and is a quick, non-invasive way to check your risk of Peripheral Artery Disease (PAD).

  • *NEW: Diabetic Foot Care - This service will be provided by a certified LPN and will be offered one day a week at:
    - Associate Medical Clinic (Whitecourt)
    - Life Medical Clinic (Whitecourt)
    - Edson MRPCN office
    - Mayerthorpe (location to be determined). 

     There is no charge to the patient for the service, but there is a one-time fee to the client of $20.00 for personal instrument packs. A doctor's referral is required.
