The Registered Social Worker’s primary function is to work with patients to determine, advocate for, and attain appropriate community resources and programs. The Social Worker will collaborate with Primary Care Physicians, their clinical staff, and other Primary Care Support team members to deliver and support through a patient-centred approach.

Duties and Responsibilities:
• Provide appropriate mental health services/Psychosocial intervention based on evidence-based practice standards.
• Intake screening and preliminary assessment including referrals, consultation, and liaising with PCN providers, family physicians, community partners Measure clients’ progress regularly and adjust treatment accordingly.
• Lead patient education programs (e.g., depression or anxiety groups).
• Assist patients to access community resources related to, but not limited to income support and transportation.
• Works with patients to build capacity to self-manage their conditions and circumstances.
• Establish treatment goals, priorities, and plans.
• Makes appropriate referrals to other services and resources.
• Collaborates with clients on how to manage their conditions independently and help them prevent avoidable recurrences or complications.
• Actively participates in program development and quality improvement activities.
• Adheres to Canadian Association of Social Workers’ code of ethics and guidelines for ethical practice.

Required Qualifications:
• Registration with applicable regulating body/association.
• Have a minimum of two years of Mental Health and Social work front line experience
• Experience in a health care setting is considered an asset.

Position Summary: Chronic Disease Management Nurse
Temporary 1-year fixed-term contract – .75 FTE

The Registered Nurse (RN) position will work collaboratively within a family practice environment to enhance the delivery of primary care services in alignment with the Patient’s Medical Home Model of Care.

The RN will provide comprehensive nursing services in Edson. The RN will work to the full scope of practice with an emphasis on providing comprehensive care, education, and guidance to patients/families.

The RN will assess, manage, and support patients and will work with designated family physician(s) to develop and implement treatment plans and provide follow-up care.

The role will target one or more patient populations:

  • Patients with chronic diseases (Diabetes, COPD, Cardiovascular Disease, Renal Disease, etc.)
  • Footcare
  • Seniors Health / Assessments

Other:

  • INR monitoring and management
  • aBPM knowledge and performance of exam
  • COVID monitoring and follow up
  • Target population: patients with chronic diseases (Diabetes, Hypertension, COPD, Cardiovascular Disease, Renal Disease, etc.)
  • Provide patient-centred health teaching with respect to chronic disease
  • Complete outreach screening for indicated patients
  • Assist with following up on medication recalls new screening guidelines, etc.
  • Coordinate and follow up on referrals to community agencies and supports

Required Skills & Abilities:

  • Fluency in English language (written and verbal)
  • Computer skills: email, MS Word, MS Excel, scheduling
  • Effective teaching and communication skills
  • Ability to recognize a change in a patient’s health status and determine when to collaborate with family physicians
  • Ability to effectively utilize Electronic Medical Record (EMR) software
  • Ability to maintain accurate records in line with practice policy
  • Ability to make evidence-based decisions in accordance with best practice guidelines
  • Knowledge of privacy legislation and associated obligations within a health care setting
  • Ability to build collaborative and respectful relationships with patients, families, and other healthcare professionals
  • Awareness of the Patient’s Medical Home Model of Care, social determinants of health, and principles of self-management
  • Ability to occasionally work outside of normal business hours
  • Use of vehicle for business use will be required which necessitates possession of a valid Alberta driver’s license and access to a motor vehicle insured for work-related travel

Required Qualifications:

  • Registered Nurse (Baccalaureate degree or diploma)
  • Registered with College and Association of Registered Nurses in Alberta (CARNA) and in good standing
  • Current cardiopulmonary resuscitation (CPR) certification – BLS for Health Care Providers
  • Current Criminal Record Check and Vulnerable Sector Check
  • A combined minimum of 5 years of primary care, acute care and chronic disease management nursing experience preferred
  • Experience working in a patient care team preferred

Position Summary: Part-time- Temporary, Panel Manager

Location: Mayerthorpe Alberta

The Panel Manager plays a key role in implementing panel processes and improvement initiatives in MRPCN Physician Clinics. The Panel Manager works in conjunction with the Physician and Health team to improve efficiencies in the office, increase access for patients and enhancing the patients Primary Care experience, improving clinical outcomes and the patient’s quality of life.

Major Duties & Responsibilities:

  • Working with physician practice(s) to maximize panel management initiatives within the established timelines.
  • Assisting the practice(s) to better understand their current access to care, their processes for patient attachment, and their model of care delivery.
  • Using the practices EMR to support and maintain the patient panel and to guide clinical care improvement based on accurate EMR data.
  • Accurate compiling and reporting of patient panel data to meet panel management goals and outcomes as required.
  • Continually updating one’s own knowledge of current screening and prevention guidelines in relation to panel management goals.
  • Working with the clinic team to document a panel and verification process.
  • Promote an understanding of MRPCN services and benefits.
  • Promote routine and ongoing updating and maintenance of patient panels/registries.
  • Aid in the implementation of ASAP, including clearing backlog and setting up new processes for screening.
  • Establish and maintain recall systems with the clinic team to ensure patients due for tests to monitor chronic conditions or preventive health screening manoeuvres are flagged, notified and appropriate follow-up ensues.
  • Use the EMR to create chronic disease registries or other target groups.
  • Supporting the PCN Nursing & Clinical teams in Panel Management.
  • Completing patient-specific forms and referrals in relation to Alberta Screening & Prevention.
  • Performing other related duties as required including participation in special projects as required.

Licensed Practical Nurse (Chronic Disease Management)

We are looking for a creative, caring and enthusiastic LPN to work with physicians and their staff to provide collaborative health care to the Mayerthorpe community.  This position works out of the Mayerthorpe Clinic.

Summary of Primary Functions

The role of the McLeod River Primary Care Network Licensed Practical Nurse is to work collaboratively within a family practice clinic to enhance the delivery of primary care services to patients across all age groups.  The nurse will work within a designated scope of practice to provide care to meet the needs of patients within a variety of clinical conditions.

Requirements

  • Successful completion of an accredited Practical Nurse diploma program
  • Current CLPNA registration
  • HCP-CPR and BCLS preferred
  • Two-Three years or more of related nursing experience preferred
  • Willingness to work in a dynamic environment that requires flexibility and balancing of competing priorities
  • Clear criminal record check
  • Valid driver’s license and reliable vehicle
  • Experience in developing priorities with variable workloads
  • Initiative, accountability, sound decision-making skills
  • Experience working in a multidisciplinary team
