About The McLeod River PCN

Established in 2010 through a trilateral agreement between the Alberta Medical Association, Alberta Health, and Alberta Health Services, Primary Care Networks (PCNs) are an extension of a family physician’s services. McLeod River PCN works with local primary care physicians in Whitecourt, Edson, Fox Creek, and Mayerthorpe in the delivery of primary healthcare services. Our team focuses on health promotion, disease and injury prevention, and care of patients who are medically complex or who have a chronic health condition.

Position Summary

The Family Practice Obstetrical Nurse (FPN) works collaboratively within a family practice environment to enhance the delivery of prenatal services. The FPN works to full scope of practice providing comprehensive care to patients of the McLeod River PCN. The FPN aims to increase continuity of care and improve accessibility of health services for patients. The FPN coordinates care, assists the physician with prenatal checks, and provides education and support for patients during and after pregnancy.

What you will do...

General

  • Provide comprehensive team-based care under the supervision and leadership of the primary care physician(s)
  • Coordinate and manage prenatal patients’ care within professional scope of practice while complementing care offered directly by physicians
  • Facilitate non stress tests
  • Facilitate referrals to specialists.
  • Support development and monitoring of treatment plans and coordinate care for patients
  • Utilize a patient and family-partnered care approach to care delivery which ensures that patients/families are partners in their health care planning and treatment
  • Facilitate continuity of care between the patient, physician, and other members of the health care team
  • Collaborate and communicate effectively with primary care physicians, PCN team members, health care professionals from external agencies, and contacts at other health/social care organizations
  • Provide continuity for patients, their family, and the primary care physician during transitions of care
  • Integrate best practices in maintenance and management of patient panel(s)
  • Maintain professional requirements, current knowledge and skills and collaborate with the Physician(s), and McLeod River Primary Care Network to identify professional development and training needs
  • Utilize quality improvement activities to improve identified gaps or knowledge in clinical care
  • Act as a Practice Facilitator for the participating clinic.
  • Participate in evaluation and reporting activities related to the clinical role
  • Assist in developing clinical policies and protocols
  • Attend relevant PCN education and training opportunities, interdisciplinary team meetings, and nursing meetings as required
  • Other duties as required

Clinical

 

  • Conduct assessments, initiate, and implement management strategies, and provide appropriate follow-up according to practice protocols
  • Document patient health history, assessment, treatment information, reports, and referrals in the EMR
  • Review and order lab work as directed by physician and follow up with patients as needed
  • Assist with medication management as directed by primary care physician
  • Provide opportunistic screening, outreach screening, and follow up
  • Coordinate routine screening for patients
  • Organize consultations and coordinate patient care/services with other team members and community agencies
  • Evaluate and document patient response to treatment and revise the treatment plan based on reassessment of patient’s condition, progress, and physician direction
  • Other duties as required
What you need to succeed...
  • Fluency in English (written and verbal)
  • Computer skills: email, MS Word, MS Excel, scheduling
  • Effective teaching and communication skills
  • Ability to recognize change in a patient’s health status and determine when to collaborate with family physician
  • Ability to effectively utilize Electronic Medical Record (EMR) software
  • Ability to maintain accurate records in line with practice policy
  • Ability to make evidence-based decisions in accordance with best practice guidelines
  • Knowledge of privacy legislation and associated obligations within a health care setting
  • Ability to build collaborative and respectful relationships with patients, families, and other healthcare professionals
  • Awareness of the Patient’s Medical Home Model of Care, social determinants of health, and principles of self-management
  • Ability to occasionally work outside of normal business hours
  • Registered Nurse or Licensed Practical Nurse
  • Bachelor of Science in Nursing or diploma in Nursing
  • 3 years of community-based health experience
