About The McLeod River PCN

Established in 2010 through a trilateral agreement between the Alberta Medical Association, Alberta Health, and Alberta Health Services, Primary Care Networks (PCNs) are an extension of a family physician’s services. McLeod River PCN works with local primary care physicians in Whitecourt, Edson, Fox Creek, and Mayerthorpe in the delivery of primary healthcare services. Our team focuses on health promotion, disease and injury prevention, and care of patients who are medically complex or who have a chronic health condition.

Position Summary

The Practice Facilitator plays a key role in implementing the Primary Care Transformation initiative by assisting clinic teams with improvement towards the pillars of the patient-centered medical home model. The role includes working with Primary Care Network (PCN) team members, physicians, and clinic staff to advance their work processes related to patient access, patient attachment, panel management, patient screening, chronic disease management, and care planning. A key aspect of this work is optimization of EMR functionality.

What you will do...
  • Engage and develop relationships with primary health care teams and family physicians
  • Facilitate awareness and understanding of the concept of the patient-centered medical home (PMH) and the associated PMH pillars related to team-based care, panel management, preventative care, and enhanced patient access
  • Assist clinic teams to identify roles and develop work processes that improve efficiency and patient-centered care
  • Support ongoing team meetings, reinforcing improvement concepts, objectives, and methods
  • Assist with the identification and implementation of improvement projects
  • Mentor and coach clinic staff to implement projects and/or new/redesigned processes
  • Work with physicians and clinic staff to leverage EMR software and other documentation systems to support proactive patient care (i.e., preventive health screening, chronic disease registries and other care interventions)
  • Act as a resource to clinics respecting EMR optimization including providing training on advanced features and promoting process standardization and efficiency
  • Support clinic teams to gather data and report on process and outcome measures
  • Liaise with PCN support team members within the Alberta Medical Association to ensure knowledge of best practices and to attain support as required

  • Understanding of primary health care delivery organizations (including family physician practices) in Alberta
  • Respect for privacy and confidentiality and the ability to reflect these principles in work processes
  • Professionalism, diplomacy, and tact in dealing effectively with colleagues and stakeholders
  • Ability to develop rapport with physicians and other health providers
  • Demonstrates a desire to continuously improve professional skills and organization practices
  • Undergraduate degree or a diploma in a related field of study, preferably a health discipline, is preferred. (i.e., LPN, RN, MOA or other allied health professional)
  • Valid driver’s license with access to a reliable vehicle for travel
  • Experience working in primary care or community health preferred
  • Experience, knowledge or training in change management, improvement methods, and quality improvement principles and their practical application to primary health care
  • Previous experience as a coach/mentor and/or facilitator and experience working with family physicians, primary care teams, and/or other primary care practices is preferred
  • Advanced training in use of Electronic Medical Records programs (e.g., Telus Med Access, Telus Wolf, Telus PS Suite, Health Quest, etc.)
