MINT Clinic Access Program

Services Provided
  • Team-Based, Comprehensive Assessments: Patients are assessed and supported by a trained interprofessional team including physicians, nurses, pharmacists, social workers, and occupational therapists—all working collaboratively to create personalized care plans. Our goal in the coming months is to train additional support staff to provide in-house support. Roberta (Social Worker) is already trained.
  • Enhanced Patient and Caregiver Support: The clinic offers not only diagnostic clarity but also ongoing management plans, access to community resources, and education for caregivers.
  • Better Health System Outcomes: The MINT model has shown measurable benefits in reducing emergency visits, delaying long-term care admissions, and improving quality of life for patients and their families.
