About The McLeod River PCN

Established in 2010 through a trilateral agreement between the Alberta Medical Association, Alberta Health, and Alberta Health Services, Primary Care Networks (PCNs) are an extension of a family physician’s services. McLeod River PCN works with local primary care physicians in Whitecourt, Edson, Fox Creek, and Mayerthorpe in the delivery of primary healthcare services. Our team focuses on health promotion, disease and injury prevention, and care of patients who are medically complex or who have a chronic health condition.

Position Summary

The Nurse Practitioner provides care and management of patients living with chronic disease and multiple co-morbidities. Some travel within the McLeod River PCN catchment area may be required.

What you will do...
  • Promote an understanding of the Primary Care Network service and benefits for all stakeholders in support of the Patient’s Medical Home model of care
  • Participate in the care of individual patients, practice populations, and associated communities
  • To assist in the development and delivery of chronic disease management and health promotion programs and provide appropriate care and treatment according to currently accepted evidence-based practice.
  • To provide restricted activities and care within the full scope of practice as defined under the Health Professions Act (HPA)
  • To make professional, competent, and autonomous decisions with patients as a member of the multidisciplinary team
  • To act as a nursing resource to other multidisciplinary team members, healthcare institutions, and community agencies or programs

  • Ability to make evidence-based decisions
  • Ability to monitor clinical standards
  • Current Registration & Practice Permit as a Nurse Practitioner with the College and Association of Registered Nurses in Alberta (CARNA)
  • Eligible for registration as a prescriber with Tracked Prescription Program (TPP) Alberta
  • Successful completion of an approved NP program at a Master of Nursing level or equivalent
  • Experience working within a primary care setting preferred
  • Current CPR (submitted yearly)
  • Must have own vehicle and valid driver’s license
